Case Manager (PT)

Chesterfield County is seeking a part-time Case Manager to provide services to adults with intellectual disabilities. Functions include assessing consumers' strengths and needs; developing individualized service plans; completing written service assessments; linking consumers with other community resources; monitoring sufficiency of services and goal attainment; collaboration with other professionals. Successful candidate will possess a bachelor's degree in a human services field with specialized coursework in case management skills and two years of experience working with people with disabilities; or an equivalent combination of training and experience. Must meet the criteria for a Qualified Intellectual Disability Professional (QIDP). Computer skills and the ability to type required. Must be knowledgeable in progressive case management, state and federal regulations for community and ICF services, and computer skills necessary to enter data into an electronic medical record. Good driving record required. Must not reflect a total of six or more demerit points within the twenty-four months preceding the anticipated hire date, or one major violation of six demerit points within the preceding thirty-six months.
