Hourly Range: $21.49 - $25.30

Chesterfield County’s Department of Mental Health Support Services is currently seeking a part-time Case Manager to provide services to adults with intellectual disabilities. Functions include assessing consumer's strengths and needs; developing individualized service plans; completing written service assessments, linking consumers with other community resources; monitoring sufficiency of services and goal attainment; collaboration with other professionals. Successful candidate will possess a bachelor's degree in a human services field with specialized course work in case management skills and working with people with disabilities with two years of experience in a human services field; or an equivalent combination of training and experience. Must meet the criteria for a Qualified Intellectual Disability Professional (QIDP) designation. Computer skills and the ability to type required. Good driving record required. Must not reflect a total of six or more demerit points within the twenty-four months preceding the anticipated hire date, or one major violation of six demerit points within the preceding thirty-six months.
