Mental Health Support Services

Be a member of a team at Chesterfield County's Department of Mental Health Support Services providing integrated care to adults with serious mental illness. Assist with identifying individuals with chronic or significant health conditions at high risk needing assistance with managing their illness. This includes reviewing prior medical records, assessing physical and mental health status, and working collaboratively with other treatment providers, including the development and implementation of a Care Plan. Services to be provided at home, office, or other community-based location. Obtain vital signs, provide educational interventions on self-management, lifestyle changes, supplies and tools necessary to manage diseases. Monitor factors affecting health and assist individuals with goal-setting to achieve behavioral change. Provide follow-up during and after hospitalizations and emergency room visits. Make referrals to appropriate agencies and services. Keep the team updated on patients' conditions and circumstances. Educate Mental Health providers on physical illnesses and interventions. Successful applicant will be licensed as a Registered Nurse in Virginia and eligible to practice. Bachelor's degree in nursing preferred; two years of psychiatric nursing experience with seriously mentally ill adults; or an equivalent combination of training and experience. Experience in a physical healthcare setting; strong written and verbal communication skills; teamwork orientation; ability to follow through with delegated tasks and accountability; resourcefulness in problem solving; proven organizational skills; proficiency in the use of an electronic record; ability to demonstrate excellent customer service and strong cultural competency skills; experience in community-based settings, including residences, doctors' offices, and other locations; ability to adhere to strict confidentiality guidelines. Good driving record required. Must not reflect a total of six or more demerit points within the twenty-four months preceding the anticipated hire date, or one major violation of six demerit points within the preceding thirty-six months.
