Case Manager - Assertive Community Treatment
Compensation: $39,936 to $41,808 Annually

Job Duties:
Will assist in the delivery of support service to improve the functionality of clients with a severe and persistent mental illness or a co-occurring substance abuse issue in the community.
Will provide case management and complete the necessary documentation.
Will be part of the on call clinical treatment team and will rotate 24 hour on-call duties with team members (advertised salary includes a 20% on-call pay stipend).
Minimum Qualifications:
- Completion of a Bachelor’s Degree in Social Work, Psychology, Mental Health or related field or Counseling (SST-2 @39,936 + benefits) OR
- Completion of a Bachelor’s Degree in Social Work, Psychology, Mental Health or Counseling or related field and one year experience working with adults who have a SPMI diagnosis in a social services delivery setting (SST-3 @ 41,808 + benefits).
- Must be willing to take rotating 24 hour on-call duties.
Any applicant must not have six (6) or more points against their driver's license or a suspended or revoked driver's license within the last five (5) years.
