Job Description:
Position Definition and Summary
Under the director of the TFC State Director and Lead Clinical Specialist, the Clinical Specialist providers case management and supportive services for clients placed as foster children in our licensed foster homes. This position not only provides all case management services for our foster children but is responsible for maintaining all required documents in the client's chart. This position is on-call to respond to crisis and emergency situations on a rotating basis.
Position Expectation and Essential Functions
- Supervises, through face to face visits and other forms of communication, clients and foster parents to ensure the quality of care requirements are met according to Virginia Department of Social Services (VDSS) licensing standards and the Treatment Foster Care (TFC) level the child is placed on.
- Complete written reports and assessments within the required timeframe determined by VDSS licensing standards and Extra Special Parents
- Document all observations, progress and concerns in the client's chart; update on-call staff of any potential crises
- Assist the client and foster parents in achieving the client's needs and goals.
- Coordinate all family visits, supervised and unsupervised, with the foster family and referring agency
- Attend all mandatory meetings for your clients such as FAPT, court hearings, VEMAT assessments, IEPs, treatment team meetings, etc.
- Coordinate with the referring agency regarding the client's discharge goals.
- Provide transportation to clients as needed. Ensure all vehicle safety and traffic laws are followed at all times.
- Attend all ESP team and department meetings
- Serve as the on-call staff member on a rotating schedule providing 24/7 support to our families and clients . Receive after hours/emergency referrals for placements within our TFC program. Staff these out with the manager on-call
- Assist in crisis management
- Perform all other duties as assigned
Education:
A Bachelor's degree in social work or a field related to social work (psychology, sociology, etc.) and a minimum of one year of experience providing case management services OR a bachelor's degree in any field and a minimum of two year experience providing case management services is required
If applicant has a Master's degree in social work or a field related to social work, their student placement may count towards the one year of experience.
Other Requirements:
Must have a valid driver's license.
Salary Range: $39,000-48,000/year
