SUBSTANCE USE DISORDER CASE MANAGER
Goochland Powhatan Community
Services is seeking a SUD Case Manager to serve adolescents and adults living with substance use disorders who are seeking treatment, with a special emphasis on opioid dependence.
This position works closely with therapists, peers, nurses, and psychiatric providers within the agency and as well as community providers. These services utilize a team approach and are coordinated with community partners to meet the needs of the consumers in recovery. The case manager should be familiar with clients’ experiences and be acquainted with resources, particularly for those recovering from opioid dependence. The case manager works with clients to set reasonable goals and help them access the chosen services/supports. Duties include completing psychosocial needs assessments, developing Individual Service Plans (ISP), monitoring
sufficiency of services and goal attainment, collaborating with collateral partners who provide a
variety of care, assisting in medication assisted recovery, providing education, scheduling
appointments, making referrals as needed, documenting in an electronic health record, monitoring
treatment compliance, linking clients to community recovery supports and meeting individuals
face-to-face in the community and their homes.
The qualified candidate must have good oral/written communication skills and strong
interpersonal skills. Current valid driver’s license and good driving record required. Bachelor’s
degree required. CSAC preferred. Familiarity with MAT for opioid dependence, a plus.
Salary is commensurate with experience and qualifications, starting at $50,000/year.
