Position Summary:

The Ryan White Medical Case Manager has as their objective to provide coordination, guidance, and assistance in improving access to and retention of needed medical and support services to mitigate and eliminate barriers to HIV care services. Medical Case Manager provides comprehensive case management services to all HIV-positive Ryan White Eligible clients seen at the VCU Infectious Diseases Clinic by performing assessments of needs of referred clients and managing the care plan created.

Core Responsibilities:

• Medical Case Management

o Provide direct social work/case management services to HIV/AIDS infected individuals and their families in accordance with their medical diagnosis, psychosocial, social and financial needs. Scope of services includes screening, assessment, crisis intervention, counseling, advocacy, information/referral, and care planning and coordination.

o Formulate plan of care with multi-disciplinary teams at different sites to incorporate the physical, spiritual, and psychosocial aspects of the individual/family to promote optimum levels of wellness

o Respond to multiple referrals from community organizations, health department personnel, and community health providers to offer information regarding services

o Maintains an exemplary level of clinical knowledge and skills for direct clinical practice

o Participate in ongoing education and training programs

o Proficient in accurate documentation in patients Electronic Medical Record and Ryan White Databases

o Assures patient eligibility for grant funded services is completed and up-to-date

• Data Entry

o Collect data and enter into CAREWare data system in accordance with the time frame as stated in the CAREWare Data System Policies and Procedures as set forth by the Data Manager

• Quality Management

o Participate as required in Quality Management and Quality Improvement activities

Minimum Hiring Standards:

o 3 years of Case Management experience in healthcare delivery system

o Knowledge of Medicare, Medicaid, and other state or federal funded assistant programs for eligible individuals

o Knowledge of HIPAA and ability to implement privacy and confidentiality practices

o Excellent customer service and interpersonal skills

o Excellent time management and organizational skills

o Ability to demonstrate superior teamwork behaviors

o Ability to work independently

o Proficient in MS Office and data entry programs

o Sensitive to confidential health information and HIV/AIDS related issues and controversies

o Outstanding communication skills, oral and written

o Cultural competency in working with a diverse population

o Demonstrated experience working in and fostering a diverse faculty, staff, and student environment or commitment to do so as a staff member at VCU

Preferred Qualifications:

o Knowledge and Experience with the Ryan White grant program
